PREPARE FOR VISIT

BELOW ARE SOME IMPORTANT STEPS TO HELP YOU PREPARE FOR YOUR VISIT:

• Call 985.449.0944 to discuss your child’s needs or INQUIRE ONLINE

• CPT will help you to verify insurance benefits and discuss payment options with you

• Fax copy of insurance card to CPT at 985.449.0945

• Obtain prescription from pediatrician or attending doctor for child to be treated and evaluated at CPT. (Diagnostic codes are very helpful)

• Set up Evaluation appointment

• Download and Fill Out NEW PATIENT PACKET and bring with to Evaluation. In order to open file, you must call 985.449.0944 for password or email us at info@therapyforpeds.com

• In-clinic Evaluation

• Begin therapy sessions
